Statewide Programs
​for Individuals with Disabilities

Rhonda had to stop working after her car accident left her permanently disabled. Without a steady income, she knew she needed to find support. Learn what the Area Agency on Aging was able to connect her to.
​The Area Agency on Aging of Pasco-Pinellas is the initial starting point to join the waitlist for the Statewide Medicaid Managed Care Long Term Care program (SMMCLTCP). This program includes not only nursing home services, but also provides care at home and in Assisted Living Facilities through a managed care system. The program is open to seniors and adults age 18+ who have a disability and are Florida residents

​If you are interested in inquiring about the program, these are the steps you will take:

​​Call the Helpline and discuss your situation with your Helpline representative. If appropriate, the representative will then have an Intake team member contact you to screen you for long-term care services. Once you are screened, an Agency team member will explain how to access services, including Medicaid long term care services. You may be placed on waitlists for any eligible services that do not currently have any available funding/open slots.

​​If and when funding & space is sufficient to allow a person to enter the SMMCLTCP program, the ADRC staff will contact you to explain the enrollment process and guide you through the steps that must be taken to determine financial and medical eligibility.

​Once you are determined to be eligible, the Agency for Health Care Administration will send you a welcoming packet about contacting the Enrollment Broker and choosing one of the Managed Care Organizations.
PUBLIC RECORDS
F.S. 119 and the Florida Constitution provide a right of access to the public records of AAAPP pursuant to F.S. 20.41. Certain records are exempt or confidential and exempt from disclosure.  Anyone can request public records from the Area Agency on Aging of Pasco Pinellas. The requester is responsible for any cost of providing the public records, which includes staff time, cost of copies and other costs that are associated with the request as set forth by law.  To make a request, please send an email to [email protected]

AAAPP’s services are available to all eligible individuals in Pasco and Pinellas counties and AAAPP complies with all legally mandated non-discrimination policies.  AAAPP strives to provide services through a lens of inclusivity by making sure underserved communities are aware of the services available and providing the requisite assistance in linking eligible individuals to available services.  The Board of Directors, Advisory Council and staff will be representative of the seniors, adults with disabilities and caregivers we serve. We welcome the unique perspectives of all persons in our quest to fulfill our mission.
