Community Care for the Elderly

Focuses on helping functionally-impaired elderly persons remain in their homes or the home of a caregiver. Core services of the program include respite care, homemaker, chore, home health aide, personal care, adult day care, medical transportation, home nursing, home-delivered meals, emergency alert response, physical therapy, speech therapy and occupational therapy. Also includes the Better Living for Seniors short term case management.
